Job description

The Integrated Transfer Team is seeking a highly motivated and enthusiastic nurse to join our new team.

This is an exciting opportunity to become part of our integrated service, which features many new initiatives designed to challenge and develop our staff.

The ideal candidate should demonstrate strong communication and negotiating skills and be able to work closely with members of the multidisciplinary team (MDT) and other stakeholders to facilitate timely and safe discharges, thereby supporting patient flow within the hospital.

You will provide clinical leadership, expertise, and advice on discharge processes, quality, and safe patient flow. The post holder will conduct initial proactive assessments for inpatients to ensure safe and effective discharges and help prevent avoidable readmissions.

The Key Aims Are As Follows
  • Deliver a coordinated approach to assist patients in identifying needs at discharge, avoiding unnecessary delays in transfer of care.
  • Collaborate with external and internal partners to ensure smooth hospital transitions and reduce readmission risks.
  • Provide transparent information and expert advice to patients, carers, and professionals.
  • Report all delayed transfers of care, including sitrep and non-sitrep cases, according to local standards.
  • Act as a leader for palliative care discharges.
  • Confidently serve as a Trusted Assessor and role model within the Integrated Team.

We seek someone committed to providing the best care for patients in the later stages of hospitalization and who maintains professionalism aligned with Trust values.

The team operates from 08:00 to 18:00, seven days a week.
