Minimum Requirements
The Transfer Coordinator assesses, plans, develops, implements, and evaluates patient transfers within the system network. They coordinate with a multidisciplinary team to ensure timely patient placement and transfers, communicate capacity management plans, and possess clinical knowledge and critical thinking skills to meet care standards. Must work effectively in a fast-paced environment with a positive attitude towards all professional contacts.
