Job Responsibility:
Provide an evidence-based specialist nursing service to a cohort of patients referred to the specialist service. This includes:
  • Delivering care through face-to-face consultation or telephone communication
  • Demonstrating advanced practice skills in holistic assessment, planning, implementation, and evaluation of care
  • Managing a cohort of patients and prioritizing workload to meet patient and service needs
  • Undertaking clinical competencies relevant to this specialist service
  • Assessing and effectively managing individual psychological reactions to diagnosis, treatment, and associated side effects
  • Facilitating and coordinating the process of informed choice for patients in the provision of treatment and care
  • Acting as a patient advocate to guide treatment and care decisions

The successful candidate will also advise on complex disease and symptom management, evaluate response to interventions, and act as a resource for healthcare professionals working within the Trust and primary care.

Key Responsibilities:

  • Co-ordinate the patient care pathway, ensuring timely referrals are made to members of the multidisciplinary and treatment teams
  • Demonstrate advanced empathetic interpersonal and communication skills in supporting, informing, and advising patients and carers
  • Effectively and sensitively communicate highly complex and sensitive information to emotionally distressed patients and family/carers
