Overview

Trainee Advanced Clinical Practitioner, Care of the Elderly / Frailty. The closing date is 24 October 2025. The successful post holder will be supported to undertake the MSc in Advanced Clinical Practice and upon successful completion will become a Band 8a. The trainee is required to work a minimum of 30 hours (0.8 WTE) as per Health Education / NHS England guidance. Competence in Non-Medical Prescribing will be required, if supported by the practitioner's professional regulator.

Main duties of the job
  • High profile, accessible and responsive leader with managerial and clinical responsibility for the delivery of Advanced Clinical Practice in Care of the Elderly and Acute Frailty service. Able to make informed decisions within their own speciality and scope of professional practice, involving patients and public to ensure agreed standards, targets and objectives are met.
  • The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the Care of the Elderly team to meet the needs of patients, and will play a key role in service delivery and adherence to policy and procedures. They will also be instrumental in guiding junior medical and nursing staff in relation to relevant protocols and procedures.
  • The post holder will be required to work within all areas where Care of the Elderly patients may be housed, including the Frailty Same Day Emergency Care (FSDEC), medical admissions unit (MAU), Emergency Department (ED), Care of the Elderly wards, outlying and rehabilitation wards undertaking primary and secondary assessments alongside medical colleagues, providing support and guidance to nursing and medical staff in the management of Care of the Elderly patients. The post holder must be willing to support the development of Geritaric outreach to the community to meet local needs and national/local health agendas.

  • Co-ordinate the care of Care of the Elderly and Acute Frailty Service patients throughout their pathway with the multidisciplinary team and clinical services to ensure effective management.
  • Accountable for own actions in accordance with the Code of Professional Conduct and professional regulatory guidance. Contribute to corporate objectives, acting within local, Trust and statutory guidelines and policies at all times.
  • Lead and deliver care across settings such as outpatient, inpatient, virtual clinic, ward rounds, etc.
  • Deliver clinical care according to national and local guidelines, ensuring evidence-based practice and working towards the Trust vision of a World Class Service.
  • Assess and prioritise patients within the service to maintain quality care.
  • Undertake initial assessments to determine differential diagnosis, utilise advanced critical thinking and decision-making skills, and refer to senior clinical colleagues when necessary.
  • Act autonomously to provide expert clinical care contributing to the patient’s treatment plan to meet physical, psychological and social care needs.
  • Request and interpret relevant investigations to inform the patient management plan.
  • Proactively refer to other clinicians or services to deliver care beyond current scope of practice.
  • Proactively initiate treatment plans for patients requiring intervention and ensure ethically based considerations.

Our ACP team is based between both hospital sites at University Hospital Coventry and the Hospital of St Cross.
