Job description

We are looking for trainee advanced autonomous practitioners to join our team. Are you excited by primary and urgent care? Do you enjoy working in a dynamic busy environment?

MedOCC is a primary medical care service providing two functions: one is the provision of primary care out of hours services for Medway and Swale, and the second is to provide the primary care element of the Urgent Treatment Centre at Medway Foundation Trust.

The services include General Practitioners, Advanced Clinical Practitioners/trainee Advanced Clinical Practitioners, administrative staff, and drivers.

The out of hours primary care also delivers cellulitis and DVT pathways for Medway and Swale, operating from 18:30 to 08:00 hours on weekdays, and from 18:30 Friday to 08:00 Monday, including Bank Holidays.

The Urgent Treatment Centre's primary care operates during hours including weekends, treating patients with various medical presentations referred via 111 and streamed by the UTC front door.

The clinical team operates a rostered shift system. MedOCC has multiple pathways and direct referral routes to all MCH services and partners such as SECAmb, MFT, KMPT, and Primary Care.

The Trainee Advanced Clinical Practitioner (tACP) will be supported through indirect supervision to assess, diagnose, interpret investigations, and treat patients within MCH, in line with developing scope of competence. They will monitor and improve standards of care, teach colleagues, and provide clinical leadership. They may act as an expert in their field and lead initiatives to improve standards of care and advance the ACP role.

The tACP role is service-driven and will develop around the Department of Health guidance across four domains of advanced practice: Expert Practice, Practice and Service Development, Education and Training, and Professional Leadership and Consultancy.

The tACP will develop specialist knowledge and skills to provide autonomous healthcare within their specialty through an established ACP training pathway, demonstrating competence in knowledge, decision-making, and clinical management.

While in training, the tACP will undertake an MSc in Advanced Clinical Practice, including work-based modules, and follow a three-year supervisor-supported training program aligned with national competencies.

The tACP will gain skills to undertake assessments, diagnosis, and treatment of undifferentiated conditions, developing decision-making, critical thinking, and evidence-based management skills.

Protected time will be provided for professional development, supporting staff, patients, and service users, offering clinical advice and support based on evidence-based research.

The role encompasses all four pillars of clinical practice, education, leadership, and research, tailored by clinical context and qualifications.
