Job summary

We are seeking a motivated and compassionate Trainee Advanced Clinical Practitioner (ACP) to join our dynamic Acute Frailty Unit (AFU) team. This is an exciting opportunity for a clinician committed to advancing their scope of practice while delivering high-quality, patient-centred care to older adults with complex needs.

As a trainee ACP, you will work under supervision while developing advanced clinical knowledge, decision-making skills, and leadership capabilities. You will be supported through a structured training pathway, including Master's level academic study and supervised clinical practice.

The Acute Frailty Unit provides rapid, multidisciplinary assessment and intervention for older patients presenting with acute illness, with the aim of early stabilization, treatment, and either discharge or appropriate ongoing care planning. Your role will be integral to the timely delivery of care, contributing to the prevention of hospital-associated deconditioning and improving patient outcomes

Main duties of the job

  • Perform comprehensive geriatric assessments under supervision.
  • Collaborate with the multidisciplinary team to develop and implement individualized care plans.
  • Support early decision-making and appropriate patient streaming.
  • Administer and review treatments, diagnostics, and interventions within scope of practice.
  • Participate in quality improvement initiatives and audits.
  • Engage in continuous learning and development, progressing toward ACP competencies across the four pillars: Clinical Practice, Leadership, Education, and Research.
Ideal Candidate:
  • Registered healthcare professional (e.g., RN, AHP, Paramedic) with relevant experience in acute or frailty care.
  • Demonstrates a passion for working with older adults and a commitment to holistic, person-centred care.
  • Strong communication, critical thinking, and teamwork skills.
  • Willingness to undertake ACP training and develop across all professional domains.

Following completion of appropriate underpinning theoretical content and completion of competency assessment (within scope of practice) request and / or undertake diagnostic procedures and clinical investigations related to plans of care.

Undertake and complete Non-Medical Prescribing role. When required training completed advice on medicine management issues associated with the patient specialty group. Work within Trust policy for Medicines Management.

Utilise advanced knowledge and skills relating to the speciality to provide specialist advice to other members of the multidisciplinary team on the basis of patient assessment.
