A healthcare organization in Eastbourne is seeking Discharge Co-ordinators to support safe and timely patient discharges. This role requires recent experience in health or social care, teamwork, and communication skills. Suitable candidates should be educated to GCSE level and have clinical and administration skills equivalent to NVQ3. An exciting opportunity has arisen for Discharge Co-ordinators to join our Transfer of Care Hub working with colleagues from both health & social care.
The role of the Discharge Co-Ordinator will be an integral part of the integrated discharge team who will be responsible for assisting the multi-agency teams with safe and timely discharges from the acute hospital setting.
This role will work to cover 7 days working in the gateway areas of the Trust on both sites.
We are seeking applicants with recent experience of providing direct health or social care to patients, hospital experience is desirable but not essential. Educated to GCSE or equivalent level with evidence of good literacy and numeracy, as well as good communication skills. Evidence of relevant clinical and administration / IT skills to NVQ3 or an equivalent level. NVQ 3 in Care is desirable.

You will be based at Conquest and Eastbourne Hospital, however there may be a requirement on occasion for cross-site cover.
