Job description

The dedicated team in Wrexham is looking for an experienced CAMHS Practitioner to provide care and support to individuals experiencing mental health issues. Your role involves developing meaningful relationships to understand service users' needs and offer appropriate support.

Responsibilities include, but are not limited to:
  1. Developing and delivering high-standard assessments and treatments for referred patients.
  2. Managing an agreed clinical caseload.
  3. Assessing and providing evidence-based treatment programs and discharge plans.
  4. Planning and delivering therapy services in collaboration with users, carers, and clinical teams.
  5. Developing and maintaining therapeutic activities suited to patient needs.
  6. Reviewing and evaluating treatment programs regularly with patients, carers, and clinical staff.
  7. Facilitating patient transitions through care stages, including care planning and therapy.
  8. Completing necessary documentation and reports.
Requirements:
  • Nursing Degree.
  • Registration with the NMC.
  • Enhanced DBS (consider whether on the update service for fast-tracking).
  • Relevant nursing degree.
  • Minimum of 2 years’ experience in a similar role.
  • Mandatory training certificates.
  • Full driving license and access to own vehicle (advantageous).
