Job description

The committed team in Swansea is looking for an experienced Tier 3 CAMHS Practitioner to join them, providing care to children and young people experiencing mental health issues. The role involves developing meaningful relationships to understand individual needs and provide adequate support.

Responsibilities include, but are not limited to:
  1. Developing and providing high-standard assessment and treatment for referred patients.
  2. Managing an agreed clinical caseload.
  3. Assessing and delivering evidence-based treatment programmes and discharge plans.
  4. Planning and providing appropriate therapy services in collaboration with patients, carers, and clinical teams.
  5. Developing and maintaining relevant therapeutic activities tailored to patient needs.
  6. Reviewing and evaluating treatment programmes regularly with patients, carers, and clinical teams.
  7. Facilitating patient transitions through care stages, including care planning and therapy.
  8. Completing necessary documentation and reports to maintain patient records.
Requirements:
  1. Nursing Degree.
  2. Registration with the NMC.
  3. Enhanced DBS (considering the update service; fast-track options may be available).
  4. Relevant nursing degree.
  5. Minimum of 2 years’ experience in a similar role.
  6. NMC registration.
  7. Mandatory training certificates.
  8. Full driving licence and access to a vehicle (advantageous).
