Job description

Overview

Change Grow Live is a charity dedicated to the belief that we can make a difference to our service users lives, offering support and respect in a safe environment, treating each user as an individual and working with them to find the right support and care options.

As a Family Practitioner within our Thriving Families programme, you will be providing drug and/or alcohol support to parents within a family intervention framework. You will be providing holistic support to families who are currently known to Child Protection, or Child in Need Plans, alongside a team of Social Work professionals, all aiming to keep the family unit intact. The multi-agency team you will be working closely with could include Family Help, Mental Health Practitioners, and Domestic Abuse Workers, requiring strong partnership working skills from the Family Practitioner to provide reliable wraparound support to the family.

As a Family Practitioner, you will work with adults within families to understand substance use within the household. This will include acomprehensive assessment of needs that will help to inform interventions, which willbe led by the requirements of the person accessing support. You will utilise an array of interventions to enable families to be able to consider the impacts of substance use, identify their strengths, and rebuild their connections with each other.

Additionally, you will work with a small case load on a routine basis, visiting them in spaces that work for their needs, including home visits. This will empower families to feel supported and embed the services' holistic family approach to familial substance use. Ultimately, the project aims to work holistically to support families to thrive and ultimately keep the family unit intact.

Where:Manchester

Full Time Hours: 37.5 per week

Full time Salary Range: £27,861.26 - £32,002.35 dependent on experience (based on full time hours, pro rata for part time hours)

Contract: Fixed Term Contract until 31st of October 2026 - with the possibilty to extend subject to review

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

Key points about the role:

  • Working as part of a multi-disciplinary team to provide family interventions that help to reduce parental substance misuse and the impact substance misuse has on children and young people. Types of interventions to include, whole family, time focussed interventions, group programmes and 1-2-1 psychosocial based interventions with parents who recovering from substance use.
  • Carry out comprehensive service user and family assessments to develop individual recovery plans that clearly identify how achieving each goal will enable progression in addressing substance use and improve relationships within the family
  • Supporting the effective safeguarding of families through contribution to multi-agency plans, timely identification of risks, vulnerabilities and responding accordingly with safety planning and onward referrals to the relevant services where required.
  • Have a role in building relationships internally, and with wider partnerships while promoting and embedding the services whole family approach to familial substance misuse.
  • To effectively maintain case management systems, reports, and notes in a timely and organised manner.
