Job summary

The role will be based in the Acute adult inpatient team and rotate through General Medicine, Cardiac, Surgical and Elderly Care. You will provide acute rehabilitation to maximise independence and support discharge from hospital, working closely with Therapy colleagues and the wider multidisciplinary team.

Successful candidates should be prepared for a 7-day service with regular weekend work. A valid UK manual car licence for community visits is preferred unless exempt.

Job description

The role will be based in the Acute adult inpatient team and rotate through areas including the General Medicine, Cardiac, Surgicaland Elderly Care. You will provide acute rehabilitation to maximise independence and facilitate discharge from the hospital setting. This will involve working closely with Therapy colleagues and the wider multidisciplinary team.

Successful candidates are expected to hold a valid UK manual car drivers full licence or be able to arrange alternative means to complete community visits as required, unless medically exempt.

Your life outside HHFT is important and the Trust is happy to consider applications with flexible working patterns that cater to individual requirements; this can be discussed at interview. We cover a 7 day service so regular weekend working will be required.

The role will require a highly motivated enthusiastic individual who has the ability to work autonomously and as part of a team. The successful candidate will be able to show initiative, have good communication and organisational skills, being able to prioritise a caseload in a busy acute setting.
