Therapy Clinical Lead (Band 7) - Malmesbury Community Team


Location: Malmesbury


Hours: 30 hours per week

In this vital leadership role, you will:

Main Responsibilities
  • Work with partners in nursing and residential care to ensure optimum health outcomes for residents
  • Support the community team to develop shared caseloads with primary care working in practice together
  • Understand and support the achievement of HCRG's business plan objectives and performance targets, and participate in screening and needs assessment as required
  • Supervise other health and social care professionals in health promotion activities, e.g., chronic disease management
  • Identify potential service developments, risks, and deficits, informing line management with recommendations based on specialist knowledge
  • Be accountable for practice, continuously improving knowledge and professional competence, and adhere to the Code of Professional Conduct
  • Ensure effective resource use within the Community Team, managing the budget appropriately, and participating in review meetings with the Business Manager
  • Develop and maintain positive working relationships with internal departments and partners
  • Assess physical and psychosocial needs of clients, evaluate care delivery, and discuss treatment options with generalists and specialists

The Ideal Candidate
  • Registered AHP
  • First degree
  • Experience in teaching, assessing, and applying research to practice
  • Current, valid driving license
  • Evidence of CPD
  • Knowledge of relevant legislation
  • Leadership and management training
  • Community and case management experience
  • Experience working with multidisciplinary teams
  • Strong communication and interpersonal skills
  • IT literacy and teaching skills
  • Manual handling and complex nursing skills
  • Flexible, adaptable, motivated, and able to work in other areas as needed
