Job description

Do you have an interest in developing your skills in falls prevention and management? Do you enjoy working with the older population? This is a great opportunity to join the Community Falls Service as a Band 4 Therapy Assistant Practitioner (TAP). Be part of a team who visit older people in their own home to support reducing their risk of falls and maintaining their independence. The service working hours are Monday to Friday 8am-4pm and works citywide across Leeds.As a Therapy Assistant Practitioner you will complete independent visits as well as joint visits to assist Physiotherapists in complex falls management. This role gives you the opportunity to further develop your skills in exercise provision, as you will be supported to attend and complete the Otago exercise training course. The service is multidisciplinary and you will have the opportunity to work alongside an Advanced Clinical Practitioner, Physiotherapists and Pharmacists.

Main duties of the job

You will be responsible for:Teaching, supervising, monitoring and progressing falls prevention home exercise programmes and mobility practice. This will include monitoring of vestibular exercises.Fitting and teaching the correct use of basic mobility aids and activities of daily living equipmentProviding feedback to the Physiotherapists regarding a patients progressionTaking blood pressure readings and feeding these back to the Physiotherapist or Pharmacist to reviewGathering information and liaising with the Physiotherapist should a patient report a fallYou will need to demonstrate excellent communication skills and have experience of multidisciplinary working with older people.
