I am seeking experienced Nurse Practitioners to join a Telephone Triage/GP OOH team in either Nottingham or Mansfield.

As an autonomous clinician, you will assess, diagnose, treat, and manage patients presenting with urgent primary care needs. You will work collaboratively with GPs, other ACPs, and clinical support staff to ensure safe, effective, and patient-centred care.

This is a key role within the provider's urgent care model, with a focus on clinical excellence, patient safety, and maintaining continuity of care through secure record keeping and coordination with NHS 111 and GP practices.

Responsibilities
  • Provide autonomous, evidence-based clinical assessments, diagnoses, and treatment plans for a broad range of undifferentiated conditions in adults and children.
  • Use advanced clinical skills to triage, risk-assess, and escalate care appropriately.
  • Prescribe medications and treatments within your scope of practice and in line with current guidelines and protocols.
  • Maintain accurate, contemporaneous clinical records and contribute to seamless information transfer to patients' registered GPs by 08:00 AM the next working day.
  • Actively support and uphold NHS England KPIs, including those relating to patient safety, clinical effectiveness, and experience.
  • Mentor and support junior clinicians, nursing staff, and trainees as required.
  • Participate in clinical audits, service evaluation, and ongoing quality improvement initiatives.
Requirements
  • Must have at least minor illness qualification at Level 6.
  • Relevant experience with telephone assessments.
  • LTD company pay available.
  • Must be able to commit to one shift a week on average.
