Main duties of the jobDischarge Home To Assess service runs 8am to 6pm Monday to Friday and 9am to 5pm at weekends. The post holder will be expected to contribute to the weekend working Rota.
The Trust is currently going through a transformation, therefore there may be some changes, this can be discussed in further either at interview or by contacting us beforehand.
The service covers both North and East Hertfordshire and the post holder may be expected at times to cover across localities.
You must hold a full valid driving licence and have access to a car to use regularly for business purposes (unless you have a disability as defined by the Equality Act 2010) and business car insurance.

Person SpecificationQualificationsEssential- Do you have an NVQ III and/or equivalent knowledge and experience?
- Do you have experience of working in a rehabilitation setting?
- Do you have experience of working within the NHS and/or in the Community setting?
Desirable- Have you worked as a Band 3 in healthcare setting with rehabilitation experience?
RequirementsEssential- Do you have experience using system one record keeping?
- Do you have good time management skills and ability to adapt?
- Do you have experience of ordering equipment e.g., Toileting equipment, pressure care or walking aids?
- Do you have the ability to support caseloads by driving across a variety of locations?
