Glossop PCN arelooking for a Community GP to support our multi-disciplinary team providinghealthcare to patients in our neighbourhood, this role is being developed to integratewith the new neighbourhood aims and agendas.

Main duties of the job

Glossop PCN are seeking to recruit a Community GP for our multi-disciplinary team providing the TeamUp service. TeamUp will provide care to houseboundpatients and people who need a bit more support, by utilising a full range of staff from multidisclipinary teams, joining up care in the community, either in person orremotely.

The role will incorporatethe home first, community second, admission last strategy.

This is an excitingopportunity to be at the forefront of the delivery of our new neighbourhood model, meeting the demands of the new 10 year plan. You will be instrumental in developing relationshipsand building stronger outcomes for the population. It willinvolve true MDT working, building relationships with GP practices, voluntarysectors, secondary care, community care, schools and other other neighbourhoodgroups.

This is an evolving service,so you will have the opportunity to contribute to the development of TeamUp and neighbourhood working.

The aim for the service isto ultimately provide a holistic approach to neighbourhood working, working alongside our Community Matron who has a focus on frailty and patients with multi-faceted health problems, there is also a neighbourhood approach to prevention.

About us

Glossop PCN is made up of four forwardthinking and progressive practices (population 33,500) who have developed a friendly and effective working relationships with each other.

Your employer would be SimmondleyMedical Practice, who operate as the lead practice for the Primary Care Network,and you would be entitled to NHS pension with 30 days annual leave pro rata plus bank holidays and study leave.

Working in conjunction with other Clinical Professionals you will cover service hours working across the Glossop area. The number of sessions you undertake is negotiable and will be discussed during the interview process. We will happily take a retainer GP or a newly qualified GP, to support our developing team.

Job responsibilities
  • Provide day-to-day clinical leadership for the multi-disciplinary team including debriefing after visits if needed
  • Home visiting for clinically complex cases e.g., end of life care, diagnostic uncertainty, complex prescribing decisions
  • Senior clinical triage for the Acute Home Visiting service
  • Build relationships across the extended team including, but not limited to social care, ambulance, social prescribers, community, and mental health clinicians.
  • Opportunity for involvement in service development as the team expands
  • Provide holistic comprehensive assessment and enhanced advance planning
  • Maximise best care in the patients own homes to reduce the need for hospital or care home admissions.
  • Provide end of life care in line with the patients wishes
