An established industry player is seeking a dedicated palliative care nurse to join their compassionate team. This role involves providing essential care to patients with end-of-life needs and dementia, focusing on both symptom management and family support. You'll work collaboratively with a multidisciplinary team to ensure high-quality care tailored to each patient's unique circumstances. This is a fantastic opportunity for a nurse who is passionate about making a difference in the lives of vulnerable individuals. If you have the right qualifications and a caring approach, this could be the perfect role for you.
Lister House Surgery and PCCO PCN have recently started a new Team Up Service to serve our patients who are most vulnerable or at need of joined up care. As part of this service, we are looking for a palliative care nurse to visit patients in their homes.
The palliative care nurse will provide care to patients with end-of-life needs and those with dementia or undiagnosed cognitive impairment, including a modified Admiral Nursing role.
Responsibilities include conducting home visits or seeing patients in care homes or where they are more stable in practice, assessing and managing complex symptoms, developing care plans, and offering emotional support to both patients and their families.
The role will involve working collaboratively with a multidisciplinary team to ensure high-quality, compassionate care that meets the unique needs of each patient.
The nurse will focus on both palliative symptom management and supporting families through dementia (or cognitive impairment) care challenges.

National Health Service (NHS) is the umbrella term for the publicly-funded healthcare systems of the United Kingdom (UK). The founding principles were that services should be comprehensive, universal and free at the point of delivery—a health service based on clinical need, not ability to pay. Each service provides a comprehensive range of health services, free at the point of use for people ordinarily resident in the United Kingdom apart from dental treatment and optical care.
