A local council in the UK is seeking a Team Manager to lead a Transitions Team focused on helping young people transition from Children's services to Adult Social Care. The role involves operational management, team development, and partnership working. Ideal candidates will have experience in a leadership position within social care and a commitment to supporting young people's independence.
Location: Buckinghamshire County Wide
Your role will be to lead a team dedicated to supporting young people and their families through the transition from Children’s and Education services into Adult Social Care services, ensuring that person-centred assessments and plans are undertaken in a timely way and that there is a strong focus on supporting young people to become more independent.
You will work alongside an Assistant Team Manager to provide leadership and motivation ensuring staff are able to provide flexible, efficient and high-quality services within a constantly changing environment.
Contract: Secondment/fixed term contract opportunity for 2 years.

Adult Social Care Team Managers are responsible for the day-to-day operational management of the following key areas:
A ‘mobile’ role gives you the flexibility to schedule your own diary, so you can meet with colleagues and clients in a variety of locations. You’ll access digital folders and resources on your laptop and be able to use the network wherever you are based. Working from home will be common, as well as meeting your team in the office where necessary.
As a registered Social Worker or Occupational Therapist you will bring you in-depth knowledge of legislation and statutory guidance and be familiar with working to a strength-based approach with particular experience and skills in working with young people.
As a leader you will be able to maintain a proactive culture within a team and work with colleagues and partners to support team development, design and implement new practices, and manage performance requirements.
As a person you will show resilience, persistence and a supportive and adaptable approach in working in an outcome focused way with our partners such as education, health services, community sector, housing, care providers and other stakeholders.
