Team Manager Continuing Health Care (CHC)

  • The post holder is responsible for the implementation and monitoring of Continuing NHS Health Care funding (CHC) in Care /Residential Homes and domiciliary settings in accordance with the national Framework for CHC and NHS Funded Registered Nursing Care (FNC).
  • The role involves extensive joint working with Conwy/Denbighshire Local Authorities to ensure patients funded by BCUHB receive a high-quality and safe service.
  • The post holder will coordinate and scrutinise CHC applications, attend and participate in Multi Disciplinary Team (MDT) meetings within the Central Division as required.
  • Manage the local resolution process for the OPMH Team, liaising with families to discuss panel outcomes when individuals challenge eligibility decisions.
  • Handle the local resolution process within the CHC Team for Inter Agency Disputes, liaising with the relevant Team Manager from the Local Authority regarding eligibility disputes.
  • Collaborate with statutory agencies on safeguarding issues and attend POVA/Strategy meetings.
  • Work collaboratively with Community Psychiatric Teams and Discharge Liaison Teams to ensure safe and timely patient discharges/transfers.

The ability to speak Welsh is desirable; Welsh and/or English speakers are equally encouraged to apply.

Main duties of the job
  • Work across professional and organisational boundaries, especially with Independent Care Homes, Social Services, other Health Boards, and Primary Care Trusts during patient transitions.
  • Develop and maintain strong relationships within the statutory, voluntary, and independent sectors, including healthcare professionals, GPs, and Local Authorities across BCUHB.
  • Promote excellent customer care throughout the CHC process, resolving concerns where possible, and managing complex, sensitive, and contentious information with professionalism.
  • Negotiate with clients, families, and the public, providing complex information in emotionally charged situations, especially regarding CHC applications/reviews with financial implications.
  • Maintain accurate case records and ensure clear, sensitive communication of panel decisions to clients and families within the required timescales, following the National Framework for Implementation in Wales.
