Overview

The London Borough of Sutton is seeking a Team Manager to lead the Hospital Pathway Team within the Hospital Pathway and Reablement (START) Service. The role is based on-site at St Helier Hospital in the Transfer of Care Hub (TOCH) alongside health partners, as a single point of referral for all hospital discharges. This is a 12 months fixed term contract with the potential to become permanent. A Welcome Bonus of £1000 is available, paid in the first month\'s salary.

About the Service and Team

The START Service facilitates safe and timely discharges from hospital to home. Our multidisciplinary team collaborates with NHS colleagues to deliver holistic, personalised rehabilitation care plans, empower residents to regain independence, and engage families and carers to support recovery at home. Our objective is to prevent readmissions while promoting overall well-being and community integration.

About the Role

Join Adult Social Care as a Team Manager to lead and facilitate the hospital discharge service. You will work on-site at St Helier Hospital within the TOCH, collaborating with health partners as a single point of referral for discharges. This role is initially a 12-month fixed-term contract, with the possibility of permanency in the near future. Your responsibilities will include:

  • Lead and manage the hospital pathway team on-site, providing clear direction to ensure effective and timely discharges from hospital.
  • Collaborate with healthcare professionals and families to develop comprehensive discharge plans.
  • Oversee the assessment of social care needs, ensuring timely and appropriate interventions.
  • Monitor and evaluate the discharge process, identifying areas for improvement and implementing best practices.
  • Foster a positive and inclusive team culture that encourages continuous learning and professional development.

Reporting directly to the Head of Service and working alongside another Team Manager, you will play a pivotal role in ensuring person-centred care from admission to discharge, leading a skilled team, and collaborating with external agencies to achieve optimal resident outcomes.

Key Qualifications and Competencies
  • Bachelor\'s degree or equivalent in Social Work; Master\'s degree and registration with Social Work England are preferred.
  • Extensive post-qualification practice experience with strong leadership and managerial skills to motivate a multidisciplinary team of social care professionals.
  • Significant knowledge of relevant legislation (e.g., Care Act 2014, Mental Capacity Act 2005) and social work theories.
  • Significant experience and understanding of hospital discharge processes.
  • Proven ability to collaborate with diverse stakeholders and community partners to achieve shared objectives.
  • Ability to work onsite 5 days a week within the TOCH at St Helier Hospital, Sutton.
