Job summary

Carmarthenshire County Council is seeking a proactive Team Manager for the Releasing Time to Care team. You will support older adults, those with disabilities and dementia, coordinating care with families and professionals to deliver timely, person‑centred interventions.

The role requires either a social work qualification or a BSc in Occupational Therapy, with knowledge of Welsh legislation and the ability to promote independence.

Qualifications

  • Social work or occupational therapy qualification.
  • Knowledge of the Social Services and Wellbeing (Wales) Act 2014.
  • DBS check will be required; basic Welsh language ability helpful.

Responsibilities

  • Deliver assessment, intervention and rehabilitation to improve function and well‑being.

Carmarthenshire County Council are looking to appoint an enthusiastic, energetic, and dynamic Team Manager for the Releasing Time to Care team who is passionate about supporting older adults; adults with physical disabilities; adults with Dementia and the families / carers of these individuals. We are an innovative, well-established integrated review team consisting of Occupational Therapy and Social Work. We strive to deliver the right support, at the right time in the right place as close to home as possible. As a team, we have an excellent reputation for our recent innovation and leadership to unite, transform and evolve our services according to the changing needs of our population.

You will have either a social work qualification or BSC Occupational Therapy qualification, be conversant with the Social Services and Wellbeing (Wales) Act 2014 as well as knowledge pertaining to relevant social work legislation and statutory obligations, role and responsibilities.

To be successful, you will have an appetite for promoting independence, know what good practice looks like and be experienced in delivering excellent outcomes and high-quality interventions for the citizen group and their families. Candidates should work well in a demanding work environment and be able to manage competing demands whilst working in partnership with citizens, their carer’s, and other professionals.

We will work together to provide further assessment, intervention, and rehabilitation to improve function, reduce risk of de-conditioning and improve the well-being of individuals within their home whilst supporting staff, providing supervision and ensuring the Local Authority provide high quality interventions with older adults and adults with physical disabilities. You will be able to manage risk confidently and promote strong partnerships across a range of multiagency partners. A commitment to relationship based practice will be essential. A Disclosure and Barring Service (DBS) check will be requested for this post. You will need a basic knowledge of Welsh. Reasonable support can be provided on appointment to reach this level.
