Job Description

Job Summary:

The Team Leader is an integral part of the support team, responsible for meeting the practical, physical, and emotional needs of the individuals we support within the service.

Through a key worker system, they will design individual support programmes and coordinate the delivery of these packages by Support Workers and themselves.

Key Accountabilities

  • Plan, coordinate, and implement the safe transition of new individuals into the service and community where necessary.
  • Liaise with families and relevant multidisciplinary teams, community groups, and voluntary bodies to promote the needs of the people we support.
  • Develop and implement individual and group support programmes to enhance independence and capabilities.
  • Conduct regular assessments and reviews using suitable models.
  • Support the physical, emotional, and personal needs of the people we support, ensuring respect, privacy, dignity, and choice.
  • Ensure medication is managed according to policies and legislation.
  • Provide leadership, supervision, and management to Support Workers, including monitoring attendance and participating in HR procedures.
  • Engage in learning and development opportunities for personal and team growth.
  • Maintain accurate records and produce necessary reports.
  • Assist in ensuring regulatory compliance.

Requirements of the Role

  • Participate in appraisals and staff reviews.
  • Model company values and behaviors.
  • Attend supervision sessions.
  • Perform duties in accordance with policies and regulations.
  • Undertake additional duties as required.
  • Work flexible hours; sleep-in payments are applicable at a minimum of £45 per night.

The duties may evolve to meet organizational needs.

Education, Experience & Training Qualifications

  • At least 2 years’ experience in a health and social care setting and a commitment to professional development, OR
  • Qualifications in Social Work, Allied Health Professions, or a Level 5 Diploma in Leadership and Management in Health and Social Care.
  • OR 1 year paid experience in HSC with a Psychology Degree and commitment to qualification.

Candidates must be prepared to complete Level 2 Safe and Effective Care Practice Diploma or hold registration with NISCC/NMC or an Allied Health Professional.

Full valid driving license and access to own vehicle are required; accommodations for disabilities should be specified.

Essential skills include communication, report writing, assessment, evaluation, problem-solving, and organizational skills.
