Team Leader – Community Planned Care

Sunderland Care and Support is one of the largest providers of social care services within the North East delivering a range of services on behalf of Sunderland City Council. We provide person-centred, innovative services that improve the lives of the people we serve. Our mission is to provide customers with the care and support they need to live happy, healthy and successful lives.

We are seeking to recruit a Team Leader into our Community Planned Care Services who is able to encourage a workplace culture based on respect and dignity for those in need of care and support. The service is community based and we provide care and support to customers in their own homes across the city, developing a wide range of skills and maintaining their independence within the city of Sunderland. Customers within our Planned Care service have a diverse range of care needs and require a wide range of support.

The Role

We are looking for experienced individuals who can lead and motivate a team of support workers to build a sense of community, encouraging our customers to have a voice, share their views, have choice, and feel at home. Qualified to at least NVQ/QCF Level 3 in adult social care or equivalent, with demonstrable experience of caring for and supporting adults. You will also have experience of supervising and leading in a care setting.

Key Responsibilities and Accountabilities
  • Provide support to an agile Service Manager and ensure the smooth and safe running of the service.
  • Manage and create monthly rotas, update and manage care plans, and supervise support workers, including inducting new team members.
  • Oversee reporting, such as medication records and health and safety checks.
  • Work jointly with other professionals to ensure customers receive support designed to meet their personal needs.
  • With the Care Co-ordinator, ensure the provision of high quality, person-centred care and support, covering practical, social and spiritual needs, in line with Sunderland Care and Support values and standards and the Care Quality Commission (CQC) requirements.
