Are you a highly motivated paediatric nurse with leadership experience, an interest in managing asthma, eczema, or constipation and a passion for providing patient care in autonomous nurse led clinics? If so, we have an exciting opportunity for you to join the Patch Children’s Community Nursing (CCN) team as a Team Leader.
The Children's Community Nursing Service includes the Children’s Hospital at home, Community nursing, PATCH and Complex Needs Nursing teams which manage a range of childhood conditions. Children benefit from the expertise of the hospital in the comfort and safety of their own home, school and a variety of other community settings. We have a dynamic team of highly skilled nurses who facilitate early discharge of children from hospital as well as prevent hospital admissions and ED attendance.
The Patch CCN team was developed as part of the Children and Young People’s Health Partnership (CYPHP), an innovative quality improvement programme aimed at enhancing the health and well-being of children in Lambeth and Southwark. Our team provides community-based care to children and young people with three ongoing conditions: asthma, eczema, and constipation.
We utilise a population-based approach to healthcare and employ progressive technology to deliver services through community clinics located across multiple sites in Lambeth and Southwark.
