About the role

Your focus as Team Leader will be to support the home management team in delivering high-quality care by providing a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff.

  • Lead the person-centred care planning process by implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, developing action plans, and ensuring action plans are completed.
  • Assist in the smooth discharge process from the home, including documentation, medication, and keeping residents’ next of kin fully informed where appropriate.
  • Monitor the wellbeing of residents, identify changes impacting care and care planning, and ensure changing needs are met with updated records.
  • Maintain safe systems of work across tasks, including moving and handling of residents, by assessing risks and prioritising safety for residents, visitors, and staff.

Qualifications
  • Hold an NVQ 3 or equivalent in care for care homes, with at least two years' experience in a senior position with the resident group, and demonstrate strong communication skills, both verbally and in writing. Also, demonstrate the ability to lead staff and act responsibly in the absence of the manager.
