Radis Community Care - Team Leader Opportunity

We have an exciting opportunity for a Team Leader to join our team based at Clarence Park Extra Care in Malvern.

About the Role

As a Team Leader, you will be the first point of contact for residents and staff onsite, as well as visiting health professionals. Your responsibilities will include:

1. Leading care shifts and managing emergencies
2. Creating and updating Care Plans, Risk Assessments, and other key documentation
3. Auditing documentation such as MAR Charts and Daily Diary Records
4. Completing staff rotas to ensure adequate staffing
5. Conducting quality monitoring checks like spot checks and carer assessments
6. Communicating with other health professionals, including doctors and district nurses

About You

• Willing to work a mix of early, late shifts, and alternate weekends
• Willing to participate in an on-call rota
• Minimum of 2 years experience in the Health & Social Care sector
