PCAS Kent is seeking a Team Leader in Folkestone to provide excellent practice leadership and support to a team of support workers. This role is essential in delivering effective home support and care services, focusing on individual well-being and outcome-based service delivery.
The ideal candidate will have strong leadership skills and relevant qualifications in health or social care and will manage performance while ensuring compliance with company policies and safeguarding requirements.

? To be responsible for providing comprehensive person-centred service delivery where the focus is outcome based and centred on the individual’s well-being.
? To support the Area Manager to lead and manage a team of support workers in the delivery of effective home support and care services.
? To work in close collaboration with the multi-disciplinary Teams (Community Mental Health Team, Social Services, Primary Care/Health Care).
1. To provide excellent practice leadership to support staff and to model the wayof working that PCAS promote.
2. To ensure that Support Plans, Risk Assessments and other associated support documents are fully completed and reviewed.
3. To ensure that Support Workers are supported to develop in line with role requirements.
4. Induction for new Support Workers, including Care Certificate assessment.
5. Supervision and Appraisal of Support Staff as delegated by the Area Manager.

6. To ensure that staff annual leave is managed in accordance with company policy.
7. To ensure that Log Notes are completed and filed in accordance with company policy.
8. To deputise for the Service Manager as required.
9. To oversee Key Workers and ensure that monthly reviews are completed, and goals set and/or reviewed as appropriate.
10. To facilitate staff and tenant meetings on the required basis and ensure minutes are produced and signed by all relevant parties.
11. To ensure that all safeguarding requirements are adhered to and met.
12. To carry out service audits as required.
13. Any other duties as reasonably expected by line manager.
14. To ensure company policies are understood and adhered to.
15. To ensure Code of Practice is understood and adhered to.
16. To undertake first line performance management activities with support staff.
17. To be the lead person for Infection Control and ensuring that services are safe.
