Liquid Personnel is currently recruiting registered and experienced Qualified Social Worker Team Leader to join a busy team at a Calderdale based Hospital Discharge Team.

Responsibilities:

1. Ensure effective screening of referrals against eligibility criteria and creative resource deployment.
2. Prioritize and assign work based on urgency and complexity, utilizing team resources effectively.
3. Manage and deploy resources within budgets to meet policy objectives and service priorities, communicating effectively with the team.
4. Review and evaluate Support Plans in accordance with guidelines, making adjustments for changing needs.
5. Implement a systematic approach to health needs assessment, planning, implementation, and review.
6. Advise Team and Service Managers on expenditure for community care services exceeding guidelines.

What we are looking for in a Social Worker:

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK licence
• Post-qualified experience
• Degree level or equivalent in Social Work
