This is a Team Leader role at Avery Healthcare, a leading provider of luxury elderly care homes in the UK. Main duties of the job

As a Team Leader at Avery Healthcare, you will be responsible for supporting the home management team in delivering exceptional care to residents. Your focus will be on implementing and monitoring person-centred care plans, assisting with resident discharges, and ensuring the well-being of each resident. You will also be responsible for leading and supporting the care team, practicing safe systems of work, and acting as a responsible leader in the absence of the manager.

Job description

Your focus as a Team Leader will be to support the home management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff.

Other responsibilities will include:

  • Leading in the person-centred care planning process by implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, developing action plans, and being responsible for completing the action plans.
  • Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed, where appropriate.
  • Leading with monitoring the well-being of each resident, being aware of any change impacting care and care planning, and ensuring that changing needs are met and written records are amended to reflect any variation.
  • Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.

  • Have an NVQ 3 or equivalent in care for care homes.
