This is an exciting opportunity to help develop our newly created Hospital @ Home Team working alongside local acute Trust's to provide early and safe discharge from hospital and hospital admission avoidance for our adult population. We are a 7 day a week service and the Team Lead is a vital part of this clinical work.

· Triage of patients referred from the virtual ward hub and from health care professionals within the community, highlighting clinical need, treatment and making referrals to Community Services as required.
· To co-ordinate the planning, development, management, and provision of a Hospital at Home service and its connections with Primary Care, the Virtual Ward Hub and the community single point of clinical access (SPCA) and to be expert in this bridging role between acute and community care.
· Working autonomously, assessing individual patient needs, initiating investigations, determining a differential diagnosis, and initiating appropriate, evidence-based treatment and care within scope of practice.
· Evaluating clinical pathways, policies, SOPS and procedures
· Providing local leadership, consultancy and advice for nurses, other professionals, patients and their carers.
· Developing close working partnerships with Community care providers; local and regional specialists; other health professionals and voluntary services involved in the patient’s care
· Lead and co-ordinate competency based training and education within the service
· Putting the patient at the centre, reinforcing the concept of a streamline patient journey, improving total patient experience. The role will involve cross-boundary and inter-agency working.
· Working with our clinical leads and integrated Multidisciplinary teams across organisations to develop the model for NHS@Home virtual ward universal personalised care within the Virtual Wards.
· Incorporating digital capabilities with the efficient use of technology to support clinical decision making
· Support a team rota to deliver 7 day a week cover
