SHYAS HELATH formerly known as S&H Youth and Adult Services Inc i is looking for one TCM program manager to join our new management team. We are located at 714 South Main Street Salisbury NC 28144. Our ideal candidate is attentive, detail oriented, and hard-working.

Responsibilities

  • Provide effective oversight, direction, and clinical and administrative supervision to Care Management team. Assume responsibility of hiring, discipline, termination, and training of staff.
  • Supervises team to ensure service delivery of care management is of quality and within the service definition. This can also include facilitating case conferences (team staffing), conducting auditing of records, direct observation of team and other activities.
  • Monitors teams to ensure active engagement with individuals through assessment in care management, care coordination, health promotion, comprehensive transitional care/ follow up, family supports and referrals to community and social support services. Ensure gaps in care are being resolved and closed. Monitoring the timeliness of care management assessments, SDoHs, ISPs and timelines for engagement.
  • Monitors levels of engagement of team for means of interactions of telephone, two-way real time video and audio conferencing or in person as defined by the service definition, BH Tailored Plan. Ensure team is adhering to the acuity timeframes of high, medium, and low for providing the care management service.
  • Collect and report outcomes to maintain quality standards and to meet the requirement defined by community resources and external participants, including but not limited to managed care organizations, accrediting bodies, etc.
  • Attend and actively participate in meetings internally and externally while representing the agency in a professional manner.

Qualifications

  • A master’s-level Licensed Clinical Social Worker (LCSW/LCSWA), Licensed Clinical Mental Health Counselor (LCMHC/LCMHCA), Licensed Psychological Associate (LPA), Licensed Marriage and Family Therapist (LMFT/LMFTA), or licensure as an RN; and three years of experience providing care management, case management, or care coordination to the population being served.
