Overview of the Job

In Lambeth, Southwark, and Lewisham, the Community TB Team LSL (CTT LSL) offers a multidisciplinary service to clients who are marginalised and have trouble accessing primary care services. They also provide care to clients who have known or suspected tuberculosis, such as homeless clients, clients with addictions, and refugees and asylum seekers.

Throughout their entire course of treatment, we offer our patients individualised, centred case management, which includes periodic follow-up in our nurse-led clinic, monitoring of their specific blood tests, CXRs, medications, treatment side effects, social issues that may impact their treatment, and mental and emotional health that may also impact their adherence to their latent TB or anti-TB treatment.

Principal responsibilities of the position
  • You will serve as a support worker for particular TB patients, making sure their treatment is completed.
  • Use a risk assessment tool to provide difficult TB patients with tailored care.
  • Determine the patient’s needs, including their financial and social needs, by conducting a comprehensive assessment.
  • Create a care plan for complicated TB patients, talk to the nurse about it, and adhere to safety procedures.
  • Build strong relationships with families and complex patients by identifying and creating adherence-related initiatives.
  • You will collaborate with the TB nurse and help the patient take and finish their TB medication. This will entail making house calls and offering directly observed treatment (DOT) in a number of locations, such as homeless shelters.
  • You will be in charge of coordinating and planning health promotion events and keeping a journal for the team and patients.
  • In order to enhance access to all basic healthcare services, you will connect with the mobile x-ray screening unit.
  • You will approach the therapeutic setting with flexibility and possess a high degree of interpersonal skills in addition to technical numeracy, literacy, and IT abilities.
Qualifications
  • NVQ at the intermediate level of education
  • Proof of GCSE-level reading and numeracy
Knowledge
  • Thorough understanding of the problems influencing the provision of health services to socially excluded groups, including those who are HIV positive, homeless, refugees, asylum seekers, and those who abuse drugs
  • Extensive familiarity with the NHS, applicable laws, and policies that impact these groups
  • Good understanding of tuberculosis, including drug resistance, co-infection (HIV & TB), adherence problems, and TB drugs
  • Equality, diversity, and inclusion: A thorough comprehension of the Trust’s diversity and equal opportunities policy
Experience
  • Extensive background in addressing the social and health care requirements of complicated clientele
  • Extensive background in implementing and assessing health promotion or training initiatives
Disclosure and Barring Service Verification
