Job summary

Are you looking for a role that provides support to patients in the community who are affected by Cancer?

This maybe the role for you! we are looking to recruit a Supportive Care Coordinator to be part of our MacMillan Team in Trafford.

You will have the pleasure of working with patients and their careers/families throughout their journey by organising, planning and offering guidance and support in a wide range of areas.

Main duties of the job

Undertake holistic assessment and together with clients plan a programme of interventions according to their identified need, whilst utilising your clinical skills and knowledge of supportive and palliative treatments n the training of volunteers, students, and other staff.

Organise and facilitate group work for the benefit of this target group of patients and carers.

Work with the Centre Manager to provide an accessible and integrated cancer information service.

Maintain accurate and up-to-date records on all clients to ensure documentation meets agreed standards.

Experience nd Knowledege
  • Palliative care nursing experience.
