Job Description

Support Worker, Wolverhampton Council, Adults 18+ Assessment Team - Must have a minimum of 2 years+ UK experience within an Adult Social Work team.

The responsibilities will include:

1. Complete reviews of adults 18+ who are in receipt of commissioned domiciliary or day services.
2. Most of the reviews will be done virtually but there will be a need to see some people face to face.
3. Gather information from any informal carers/family, care providers, and professionals involved.
4. Navigate their way around and record information including the reviews/support plans on CareFirst.
5. Manage their own workload.
6. Attend regular project meetings and provide information on their progress.
7. Present information at ‘Huddle’ meetings if they identify that a person's support plan needs increasing.
8. Liaise appropriately with colleagues and other professionals.
9. 37 hours per week (flexible by negotiation).
10. Immediate start.
11. 3 Month Temporary Contract.

The successful candidate will need to hold an NVQ3 in Health & Social Care or equivalent, have a current CRB, and be a car driver.
