Job Role

We are seeking a compassionate and motivated Support Worker to join our team, specifically focusing on supporting individuals through Transitions. This role will involve helping individuals as they move through key changes in their lives, home to independent living, or supporting people with disabilities to adjust to new routines.


Key Responsibilities
  • Provide person-centred support to individuals navigating life transitions.
  • Assist with developing and implementing tailored support plans to address unique needs.
  • Offer practical help and emotional support to enhance independence and well-being.
  • Build strong, trusting relationships with individuals, their families, and other professionals.
  • Assist with daily living tasks, including budgeting, meal preparation, and personal care as needed.
  • Promote inclusion, advocacy, and participation in the community.
  • Support individuals to develop skills that increase their confidence and self-sufficiency.
  • Encourage individuals to express their preferences, values, and needs.
