Job summary

AFG is seeking a dedicated Support Worker to help deliver personalised, outcome-focused support across a range of settings, including community-based home visits and our Independent Hospitals or Adult Care Homes.

You will support people with diverse needs, helping to tailor plans to their aspirations while accessing opportunities to undertake vocational qualifications and benefit from career development and lifestyle support services from day one of employment.
