Job summary

The Cellar Trust in Bradford District is seeking a dedicated support worker to join the Reach team. You will hold a caseload of clients with severe mental illness, delivering recovery-focused, person-centred support that empowers people to progress.

You will draw on your own lived experience where appropriate to inspire and support others, ensure accurate notes, and work with safeguarding policies and a range of health and social care colleagues to provide high-quality joined-up support.

Qualifications

• GCSE level 4 or above or equivalent experience.
• Awareness or knowledge and/or lived experience of mental health challenges.
• Experience using IT systems to record client notes.
• Experience in a multi-agency health or social care setting.
• Ability to manage a caseload and workload effectively.

Purpose of the Job

To work as part of the Reach team and alongside the Community Mental Health Teams (CMHTs) to support clients with severe mental illness (SMI) to reach their goals.

Main Duties

• Hold a caseload of clients, providing one to one support for individuals in a recovery-focused and person-centred way, which builds confidence and helps people to move forward.
• Use your own lived experience of recovery from mental health problems, as appropriate, to inspire and support others.
• Ensure that client notes are recorded in an accurate and timely manner and kept updated.
• Work in line with all relevant safeguarding policies and procedures, ensuring concerns are escalated appropriately.
• Work closely with other health and social care professionals including statutory and voluntary agencies to ensure that support is joined up and of high quality.

Person Specification

Knowledge

Educated to GCSE Level 4 or above, or equivalent experience

Experience

At least 2 years' experience of working one to one with clients who face challenges with their mental health, including with clients who have SMI.
