Overview

The Lambeth Living Well Network Alliance supports people in Lambeth experiencing mental illness. Thames Reach works in partnership with South London and Maudsley NHS Foundation Trust, Lambeth NHS Clinical Commissioning Group, Lambeth Council and Certitude to deliver mental health services as part of the innovative transformation of mental health services in the borough.

You will not accept applications where Artificial Intelligence (AI) has been used to assist with completing the competency questions.

Responsibilities
  • Contribute to effective day-to-day service delivery.
  • Conduct Initial Assessments, Needs Assessments and care plans with clients in the service and present to cases in MDT meetings.
  • Manage a caseload of clients alongside your colleagues, identifying the most appropriate treatment plan within the Lambeth mental health pathway.
Qualifications and Skills
  • Excellent communication, interpersonal, and advocacy skills, proven within a context of working with vulnerable adults.
  • The desire to work in a busy Community Mental Health team and collaborate with colleagues to achieve quick but sustainable outcomes for our clients.
  • Strong time management and prioritisation skills, and the ability to deploy these independently.
