Job description

To provide support worker services within the Uttlesford SDFS team based in Stansted.

The successful candidate will work alongside a Multi-Disciplinary Team, supporting people living with dementia and older adults with complex mental health needs.

The support worker role is diverse, including tasks such as completing physical health checks, implementing biopsychosocial care plans under the guidance of qualified MDT staff, and building relationships with patients and their families to enable them to live with dignity and positivity.

Role Summary
  1. Work independently and collaboratively as part of a multidisciplinary team, offering mental health support and treatment in the Trust's service area. The service operates five days a week, Monday to Friday, 9 am - 5 pm, delivered in patients' homes.
  2. Act as a role model within the team.
  3. Collaborate with key partner organizations, service users, clinicians, and other practitioners within the multidisciplinary team.
  4. Deliver high-quality, effective operational services to people with dementia and mental health needs, aligned with the Trust's principles and vision. The role requires flexibility and collaboration within the Integrated Community Team and the wider Uttlesford Neighbourhood team.
  5. Work with others within the organization to provide exemplary care to service users and their carers.

Successful candidates will attend a 5-day face-to-face induction at Brentwood Community Hospital, with dates to be discussed during recruitment. This will be followed by a 5-day mandatory training at EPUT.
