Job summary

Salutem Care and Education is seeking a dedicated Support Worker to provide high-quality, person-centred care to individuals we support. You will work closely with the team to enable independence, manage risks, and promote wellbeing in a safe, nurturing environment.

You will follow care plans, support health needs, facilitate reliable communication, assist with daily living activities, and ensure accurate records and DBS checks are completed as part of our safeguarding process.

• Activity Planning: Develop flexible, engaging activity plans that promote autonomy and personal growth.
• Health and Lifestyle: Support individuals in maintaining healthy lifestyles and managing their health needs.
• Communication: Facilitate effective communication and maintain detailed, person-centered records.
• Medication Management: Oversee medication processes and support individuals in self-administration.
• Safety and Environment: Maintain a safe, clean-living environment and adhere to health and safety protocols.
