Responsibilities

• Provide person-centred support to individuals, promoting independence, choice, dignity and wellbeing in line with care plans and PBS plans.
• Deliver tailored support meeting physical, emotional, social and mental health needs.
• Assist with personal care, mobility, nutrition, continence care and use of specialist equipment where required.
• Administer and manage medication safely in accordance with policies and training.
• Work collaboratively with colleagues, families, healthcare professionals and external agencies to achieve positive outcomes.
