Support Worker - Complex Care - West Sussex

Job title: Support Worker
Department: Claimont Head Office
Reports to: Clinical Operations
Job purpose: Claimont Home Care Ltd is a CQC-registered, unique service in the private sector delivering specialist mental health nursing at home, especially when inpatient admission is being considered or for inpatients treated in hospital but who wish, and are able, to be discharged home. Claimont is not an agency. We invest in people and build relationships based on mutual support and long-term commitment. The work we offer our staff can be varied and is rooted in a Patient-Centred, holistic approach. We value our nurses and support workers' skills and experience and support them with any additional training they may need.
Key accountabilities:

  1. Support clients as involved in their care, ensuring adherence to management policies, protocols, and practice guidance, updating as needed.
  2. Maintain written records of all aspects of client treatment and progress, including electronic records within 24 hours.
  3. Enable clients to access community activities and work with families/carers to support understanding of mental health issues.
  4. Maintain confidentiality and effective communication with all stakeholders.
  5. Develop and maintain clinical links with colleagues, carers, relatives, and community services.
  6. Encourage evidence-based practice and participate fully in team meetings.
  7. Report assessment findings appropriately and ensure data security and confidentiality at all times.
  8. Attend mandatory training and participate in personal and professional development.
  9. Adhere to professional standards, organizational policies, and legal requirements, including safeguarding and data protection.
  10. Promote quality and safety in all activities, ensuring compliance with CQC regulations and continuous improvement.
  11. Support a culture of equality, health, and safety, reporting incidents and adhering to policies.
  12. Participate in recruitment, safeguarding, and internal communication processes.
