Job summary

Active Care Group is seeking a Support Worker for Foxley Lane in Purley. You will deliver person-centred care, assist with daily living activities, and support residents with epilepsy and other needs.

This role requires flexible hours, including weekends and bank holidays, with a commitment to safeguarding and high standards of care. The role involves mentoring colleagues, maintaining records, and working closely with families and healthcare professionals to ensure well-being and independence

Qualifications

• Excellent communication with residents, families and colleagues.
• Good record keeping and accurate documentation.
• Ability to work as part of a team to deliver person-centred care.

Responsibilities

• Deliver person-centred care and support in line with PCPs.
• Administer medication and support individuals with epilepsy and healthcare needs.
• Provide personal care to promote dignity and independence.
• Assist with activities of daily living, mobility and community participation.
• Respond to behaviours that challenge in a calm, professional manner.
• Maintain accurate care records and ensure confidentiality.
• Serve as a key worker and liaise with families and healthcare professionals.
• Support service operations and mentor colleagues during shifts.
