Job description

This vacancy is open to applicants currently employed by Cornwall Partnership NHS Foundation Trust, Royal Cornwall Hospitals NHS Trust, or the Integrated Care Board for Cornwall and the Isles of Scilly.

We are seeking an experienced community support worker who has worked with people experiencing psychosis and other mental health conditions.

The successful candidate will work in the new "At risk mental state" (ARMS) team within the Early Intervention for Psychosis Service (EIPS).

This role involves supporting individuals to improve their mental health by working with them, their families, the multidisciplinary team, and wider community support agencies.

This vacancy covers Mid Cornwall, aligning with the area model of care.

Main duties of the job:

  1. Establish and maintain effective contact with service users, their relatives, and carers to ensure good communication.
  2. Provide information and assistance to service users and their carers regarding their rights and entitlements from the team and the wider health and social care community.
  3. Assist service users and their carers in accessing identified services.
  4. Monitor mental health and observe fluctuations, reporting changes to team practitioners to ensure appropriate services are provided and risks are managed.
  5. Encourage social and physical activities suitable to the service user's needs.
