An established industry player is seeking a compassionate professional to join their Community Mental Health Team for Older People. You will benefit from our flexible and agile working practices, which allow you to work from various locations including council buildings and your home, to suit your work, meetings, and collaboration needs.
You will be part of the Community Mental Health Team for Older People.
Your responsibilities include contributing to mental health and needs assessments of service users, providing appropriate support to service users and their carers through participation in support plans, and offering social, personal, and mental health support tailored to their needs. You will also ensure the rights of service users and their carers are maintained and contribute to the effective delivery and development of the service.
