Swanton Care is recruiting a Support Worker for a service near Seaham, Sunderland. You will fully support individuals to live as they wish, working with families and managing care plans, while acting as a Key Worker to ensure the person’s best interests are upheld.
Responsibilities include recording outcomes, safeguarding confidentiality, assisting with personal care, and medication management. DBS/enhanced checks required.
