Job summary

Swanton Care and Community is recruiting Bank Support Workers for a service near Seaham. Based in Sunderland, the role involves helping individuals live independently and supporting families in planning, as a key worker in a team.

You will support health and wellbeing, maintain dignity, manage medications, and participate in audits while promoting independence and community engagement. DBS checks apply.

Qualifications

  • Level 2 English proficiency or equivalent.
  • Care Certificate required or willingness to work towards it.

Responsibilities

  • Fully support individuals to live independently and involve families in care planning.
  • Act as a Key worker and promote teamwork in the best interests of the person.
  • Ensure health, mental and emotional wellbeing; advocate and respect consent.
  • Record outcomes in confidential care records and contribute to care plans.
  • Maintain dignity and assist with personal care as required in the plan.
  • Medication management including ordering, storing and administering.
  • Assist with audits and maintain active involvement in service upkeep.
  • Encourage engagement in activities and community involvement to boost independence.
  • Communicate with individuals in their preferred method and respect beliefs.

About The Role
  • We are recruiting for BankSupport Workers for our service near Seaham.
  • Location: Sunderland
  • Job type: Bank
  • We do not offer sponsorship
The role
  • To fully support the individual to live and enjoy the life they wish to have. Fully involve them and their families and support them in all aspects of their care and support planning.
  • Acting as a Key worker and understanding that ‘working together works best for a number of service users ensuring that the team always works in the best interests of the individual and encouraging positive behaviours across the team.
  • Ensuring the individuals needs are met to maintain health and well being, physically, mentally, and emotionally. Advocate on behalf of the individual and ensure no decision is made without them.
  • Recording and evidencing outcomes in service user records, ensuring confidentiality. Contributing to the preparation, implementation, monitoring and regular review of individual care and support plans. Giving the individual a voice that is heard and their aspirations are worked towards with them.
  • Ensuring individuals’ dignity is maintained at all times and given support with their personal and intimate care as required in their care plan. This may include bathing, toileting and assistance with dressing.
  • Medication management to include, ordering, storing and administering.
  • Undertake service responsibilities in relation to completing audits related specific to supported living.
  • Encouraging the individuals you support to engage with activities they already enjoy, and to seek out new experiences, Actively involve them in making decision about everyday tasks that will enhance their independence and engage with the wider community.
  • Empower individuals to actively achieve their goals by taking time and communicating in their preferred method, be aware of the individuals’ beliefs and cultural needs and work in an open and trustworthy manner.
  • To contribute to at least 6 team meetings each year, actively engaging in supervision and appraisal to continuously improve and develop knowledge and skill that will enhance the lives of the people within the service.
About you:
  • You will be able to demonstrate equivalent of level 2 in English and either hold or be willing to work towards Care Certificate
  • You will have worked within a team for a minimum of 2 years and have the ability to communicate effectively in English whilst having the ability to use variety of differing methods of communication according to needs of person we support
