SUPPORT WORKER

Job Purpose
Promote the mission statement and values of Harley House providing safe, sound, and supportive care and support.

To work with the people that you are paid to support to enable them to lead as independent and fulfilling lives as possible. This job description lists the main areas in which you will be expected to provide support. We may need to add more specific tasks to this based on the needs and wishes of the people. This supports their right to have control over their own lives.

Main Duties And Responsibilities
Support
  1. Follow the individual support plans and risk assessments and ensure that changes to individual needs are escalated to the team leader/manager.
  2. Support the person(s) to live the life they want to lead, make decisions, have as many choices about their lives as possible, and be supported to take appropriate risks.
  3. Make each day the best it can be by maintaining their existing skills and developing new ones.
  4. Access facilities in the local community and take part in activities outside of their home, including appointments, religious and cultural activities, and work and leisure activities.
  5. Support people with cooking, shopping, and ensuring a clean home according to individual choices.
  6. Support people to make informed decisions about their health and administer prescribed medication.
  7. Ensure good written communication on daily care records via a digital care recording system in place.
  8. Support with personal care (e.g. washing, bathing, shaving, going to the toilet) when required.
  9. Support people to achieve their ambitions as set out in their support plans.
  10. Assist with the management of finances.
  11. Take an active involvement in support plans and ensure these reflect the people we support.
  12. Act as key worker for people ensuring that their experiences and quality of life are regularly reviewed.
  13. Participate in daily handovers ensuring that you contribute key information related to the people we support.
  14. Ensure that accidents, incidents, and near-miss events are recorded and escalated to the team leader/manager.
  15. Carry out any additional tasks or duties according to your support worker responsibilities and within your scope of competence.
You Will Be Expected To
  1. Make safeguarding an absolute priority. Report concerns, poor practices, or suspected abuse always.
  2. Be a positive role model for the people we support.
  3. Act as an ambassador for the service ensuring that as a support worker, you always maintain professionalism and integrity.
  4. Refrain from befriending service users on social network sites.
  5. Treat the person(s) you are paid to support with dignity and respect.
  6. Always act in the best interest of any person lacking capacity.
  7. Be aware of, and follow the procedure for, reporting any safeguarding concerns or incidents.
  8. Keep accurate records and work with digital care records.
  9. Attend training and ensure all mandatory training is up to date.
  10. Attend team meetings as required.
  11. Attend daily handovers ensuring that key information is always communicated to your colleagues.
  12. Co-operate and attend yearly approval and supervision.
  13. Ensure the health, safety, and welfare of the people you support and your colleagues.
  14. Embrace teamwork by ensuring effective and open communication for the benefit of the people we support.
  15. Driving a mobility car is essential.
  16. Undertake any other appropriate duties as required by the person(s) you are paid to support or manage that are appropriate to your grade.
  17. Perform all duties by the organisation’s policies and procedures.
  18. Be flexible to ensure that people are supported in a wide range of activities including bank holidays, weekends, and evenings.
  19. Maintain the confidentiality of the people we support.
  20. Work weekends as the rota sets out.
  21. Participate in night shifts as required.
