At Glassmoon Services, we provide 24-hour support to individuals with autism and/or learning disabilities, including a range of needs and different presentations. Are you skilled in supporting a young person with complex needs? Can you help them build and maintain meaningful connections within their community? Can you collaborate effectively with various professionals to ensure that this young individual can achieve their desired outcomes? This role may present challenges, but it also offers great rewards. You will play a vital role in helping young individuals on their journey towards attending college, securing a job, enjoying music festivals, building relationships, and most importantly, acquiring the skills they need to become as independent as possible.

We can guarantee that no day is ever the same, but you'll work alongside a group of dedicated individuals who care deeply about their work, the people they support, and their colleagues. As a Support Worker, you'll play a meaningful role in:

  • Supporting and enabling the People We Support to live their best life, whilst delivering person-centered care, unique to each individual, aligned with our person-led support delivery model.
  • Using your knowledge, skills, and adherence to legislation, best practices, and our policies and procedures to deliver the highest quality care.
  • Supporting individuals to engage with their interests, maintain skills, and receive personalized, person-centered care.
  • Actively engaging with the people we support to foster their relationships and community connections.
  • Completing and maintaining Support Plans and other necessary documentation to ensure effective support.
  • Notifying the Team Leader/Senior Support Worker of any changes in the needs of the individuals supported, to promote their well-being.
  • Promoting and maintaining the safety and safeguarding of those in our care.
  • Helping individuals to maintain and develop relationships with family, friends, and others important to them.

Requirements:

  • Drivers preferred (all checks are fully funded by Glassmoon Services Ltd).
  • Right to Work in the UK.
  • Satisfactory Criminal Records Checks.
  • References.
  • ID & Address check.
