Salutem Care and Education’s First Key Hereford team is hiring a Support Worker to provide high-quality, enabling care at home, in the community and in supported living settings. You’ll play a key role in helping people live their best lives and focus on abilities rather than disabilities.

• Individual Outcome and Wellbeing: Implement person-centred support plans, promote independence, manage risks, encourage relationships, and ensure legal and best practice compliance.
• Activity Planning: Develop flexible, engaging activity plans that promote autonomy and personal growth.
• Health and Lifestyle: Support individuals in maintaining healthy lifestyles and managing their health needs.
• Communication: Facilitate effective communication and maintain detailed, person-centred records.
• Medication Management: Oversee medication processes and support individuals in self-administration.
• Safety and Environment: Maintain a safe, clean-living environment and adhere to health and safety protocols.
